Advancing patient-centered integrated care across the continuum of care

The combination of the Careteam platform and Trillium Health Partners’ Institute for Better Health (IBH)'s Care Connector enables a more streamlined and integrated experience for patients as they move from an acute care setting to home or community care, the standard for the newly forming Ontario Health Teams (OHTs), and regional / integrated health networks everywhere.

Mississauga, Ontario [May 24, 2023] – Careteam Technologies, a cutting-edge Canadian digital health company with a focus on integrated care, has acquired Care Connector, a unique patient-centred technology solution that was developed by the team at Trillium Health Partners’ Institute for Better Health (IBH) to improve the integration and coordination of care planning. The combination of both the Careteam and Care Connector platforms will enable a more streamlined and integrated experience for patients as they move from an acute care setting to home or community care.

Careteam, created by Careteam Technologies, is an innovative collaboration platform that brings the patient, personal support team and health care providers together with an integrated care plan, instant communications, and population health analytics to better enable care transitions and complex chronic care management in an often fragmented health care environment.

Care Connector was developed internally at THP and co-designed with more than 2,400 health care providers by THP’s Institute for Better Health to support and improve in-hospital team-based care and pre-discharge care planning and coordination. 

Together, the combined platform will create the standard for the newly forming Ontario Health Teams, and regional health networks everywhere, enabling them to deliver on the vision of truly person-centered coordinated care, and effective engagement of the patient and family.  They can start by quickly focusing on initial priorities populations, or move directly to integrating care at an overall community level. Careteam Technologies and THP are collaborating with the Mississauga Ontario Health team, with a first deployment of the combined platform focused on improving cross-continuum coordination of palliative care.

Both the Care Connector and Careteam platforms were created with generous support from the Centre for Aging + Brain Health Innovation (CABHI), powered by Baycrest, which helps innovators develop, disseminate, scale and promote adoption of promising innovations in the aging and brain health sector.
